Comments:  Microfibrillar collagen hemostat (MCH) is a topical, absorbable, hemostatic agent. When in contact with bleeding surfaces, MCH attracts platelets which adhere to fibrils and trigger platelet aggregation into thrombi. This effect is not inhibited by heparin. The platelets of patients with clinical thrombasthenia do not adhere to the hemostatic material in vitro. Do not use in the closure of skin incisions as it may impair healing of the skin edges. Toxicity:The major concern is the possible potentiation of infection. In addition, its use in adults has been associated with adhesions, allergic reactions, and foreign body reactions. MCH contains low levels of bovine serum protein (beef serum albumin or BSA) which reacts immunologically. Increases in anti-BSA titers have been observed following treatment with MCH.

Preparation: Microfibrillar collagen hemostat (Avitene) is available in a 1 gram sterile bottle. Apply with dry smooth forceps. It will adhere to wet gloves. Then apply pressure to the MCH with a dry sponge for 1 to 5 minutes.  Moistening or wetting will impair its efficacy. Do not autoclave.
